India Now Has 1.40 Lakh MBBS Seats, So Why Is Karnataka Still #1?

18 Sep, 2026

Getting one medical institute seat is the dream of millions of students, and if this dream gets fulfilled they are over the moon. Recently, The National Medical Commission's latest seat matrix, released in September 2026, puts India's total MBBS intake at 1,40,124 seats across 836 medical colleges for the 2026-27 academic year. The number is big itself. But one State always tops in intake and that is Karnataka.

For NEET UG 2026 aspirants trying to make sense of counselling strategy, this isn't just trivia — it's a pattern worth understanding.

The Numbers Behind the Headline

Karnataka now holds the largest MBBS seat count in the country. After this year's expansion, Karnataka added the highest number of MBBS seats of any state, taking its total to 15,395 — spread across 75 colleges. What's striking is that Karnataka isn't even the state with the most colleges. Uttar Pradesh has the highest number of medical colleges at 88, followed by Maharashtra with 84 and Tamil Nadu with 78 — Karnataka trails at 75.

Despite fewer institutions, Karnataka packs in more seats per college than its rivals.

So the "why" isn't about having more colleges. It's about what kind of colleges Karnataka has — and who's funding them.

The Private Sector Did the Heavy Lifting

Here's the real story: Karnataka's dominance is almost entirely a private-sector phenomenon.

Of the state's 15,395 seats, 4,400 sit in 24 government medical colleges, while 10,995 — more than two-thirds of the total — come from 51 private medical colleges. Put differently, private institutions contribute nearly 71 percent of the state's entire MBBS intake.

This year's growth reinforced that pattern. Karnataka was handed roughly 1,300 additional seats for 2026-27, and 1,150 of those went straight into private colleges — underlining the state's continued reliance on private investment to expand medical education.

New entrants like Alva's Institute of Medical Sciences in Moodbidri and the Chettinad Institute of Medical Education added fresh capacity, while established private players such as PES University Institute, Sri Siddhartha Institute, and BGS Global Institute each secured 100 additional seats.

Karnataka has essentially built a self-sustaining ecosystem where private medical education entrepreneurs keep investing, and the regulator keeps approving. Government capacity, by contrast, has been far more cautious — held back at times by infrastructure requirements the NMC wasn't willing to waive.

The South Is Quietly Running the Table

Karnataka's rise isn't happening in isolation — it's the leading edge of a broader southern surge. Even before this year's expansion, the five southern states already accounted for 38.6 percent of all MBBS seats in India. In this year's round alone, Karnataka's 1,300-seat addition accounted for more than one-third of the entire southern expansion and just over 13 percent of all new MBBS seats approved nationwide.

Tamil Nadu isn't far behind either, having crossed close to 14,000 seats with 950 new additions, 800 of them in private colleges.

This tells you something about regional patterns in medical education investment: southern states, and Karnataka in particular, have a deep-rooted culture of private capital flowing into professional education — engineering colleges built that playbook decades ago, and medical colleges followed the same template.

What This Means for NEET UG 2026 Aspirants

If you're prepping for counselling, three things follow from all this:

•More seats doesn't mean more affordable seats. Because Karnataka's growth is private-sector heavy, a large share of its "extra" capacity comes with private-college fee structures, not subsidised government ones.

•State quota matters enormously in Karnataka. With 71% of seats in private hands and most private capacity feeding into state and management quotas, out-of-state candidates chasing Karnataka seats through All India Quota will find the actual accessible pool much smaller than 15,395.

•Track the trend, not just the total. The national figure of 1.40 lakh seats is still provisional — experts believe the seat count will keep climbing before the final rounds of counselling, with colleges in Bihar, Madhya Pradesh, UP and the North-East still awaiting NMC sign-off.

The Bigger Picture

Karnataka's position at the top isn't an accident of geography — it's the outcome of a specific policy environment that has let private medical education flourish faster than almost anywhere else in India.

Whether that's a success story or a cautionary tale about the cost of medical education probably depends on which side of the fee structure you're standing on.

Either way, for the NEET UG 2026 cohort staring at counselling forms, understanding why Karnataka leads is more useful than just knowing that it does.

NEET, one of the prestigious exams of India demands lots of hard work and once you clear the exam, the story doesn't end here as landing an MBBS seat is only half the job. The rest is more important and crucial i.e locking a choice during counselling and the bond policy attached to that seat because a cheap government college can cost more than a private one if you don't know about service obligations and discontinuation penalties.
